Building Order Sets/CPOE

We're going live with Paragon in the ED next year and wanted to know more about how people are building/using order sets.  Say a patient comes in, we pull an order set and labs are ordered, can we pull the same order set once those labs are returned to select medications or treatments?  Is it even right to use the same order for that?  I want to make sure we put a process in place that won't have any bad consequences down the line that we may not know of.  Any advice is appreciated.

You absolutely can use the same order set over and over agian.  Generally speaking, for ED order sets, you may want to buld everything (mostly) with a Once frequency and a STAT priority, which is changeable by the person ordering within the order set very easily. 

Using the naming convention for the Order Set name will assist the provider in knowing which order set to choose for a specific diagnosis, groups or orders {Abdominal Pain - female, RX-anti-emetics, STAT Cardiac Orders, (which may include, meds, Labs, Imaging, nursing, etc)}.

 

There may be order sets where nearly everything is pre-selected, such as diagnosis-specific emergent orders (resp arrest, hypertensive emergency, etc) to order sets where nothing is pre-selectd and the provider chooses what she/he wants.  You can go back to the Abd pain-female order set the second or third time to add labs, diet, imaging, etc orders to the initial orders placed.
