Meritage ACO, Marin General Hospital (MGH), and other community providers have  deployed a real-time multidisciplinary mobile care navigation network that has enabled multiple caregivers, from diverse locations, to seamlessly collaborate on care, create safe transition plans, and engage patients post-discharge.

After six months of deployment, care quality process measures and efficiency measures showed significant improvement. Through real time, efficient, and effective management of care transitions, MGH was able to realize a reduction in all-cause 30 day readmissions and  length of stay while improving the patient experience related to discharge management.
